The continued escalation of pipeline failures remains an area of concern in the oil and gas industry. Process types of equipment are the most critical assets in the triangle of business operations because it contributes directly to operational costs and the company's profitability (Mutlaone, 2009). Plant availability is regarded as the most critical business objective everyone should strive to achieve for the organization's success. However, poor maintenance strategy results in safety events that endanger people and the environment. Toyota is a Japanese company that adopted the total production system. The total production system was adopted to improve the quality and eliminate and minimize defects in the production process (MacBride, 2022). In addition, unplanned maintenance reduces business profit. Moreover, operation management should reduce maintenance costs to advance people's safety, prolong the business's life span and protect the environment.
Poor risk management is the leading cause of pipeline failures in the oil and gas industry (Hopkins, 2012). Further, the slogan by engineers that “If not broken, why fix it?” indicate an attitude that reveal the importance of learning from near misses and through pipelines which are corroded. Corrosion remains an unresolved threat in the oil and gas industry.
For example, on the 20th of April 2021, at one of the plant, there was a fire and catalyst at the conical section of the hopper due to the pinhole on the plug valve body. In an integrated report for the year under 30 June 2021, it was highlighted that a few setbacks in commitment to achieving zero harm policy, occurred due two tragic workplace fatalities which were recorded respectively of the 2020 and 2021 financial years. In the same year, incidents related to fire, explosive and severity role (FER-SR) increased from 9.9–10.8% (Morgan, 2022). The company thus affected also shows commitment to the national energy sector by contributing 1.3MW to the South African grid to assist Eskom in meeting electricity demand.
The oil and gas industries must adopt the maintenance strategy used by nuclear industries to accomplish the nuclear renaissance. The Three Mile Island nuclear incident, Fukushima Daiichi and Chernobyl nuclear disasters all counted as examples. However, safety records remain excellent and the next generation of nuclear reactors were designed to avoid Chernobyl explosion disaster (Pokoha, 2018). According to (Mutlaone, 2009)the business asserts need a proper maintenance strategy to continue maintenance work at the Matumba power station that will prolong the equipment's lifespan to ensure effective operations. The state of the nation address 2023, delivered by President Cyril Ramaphosa, strictly focused on energy and minerals (Ramaphosa, 2023). In his speech, the president further articulated the Minister of Electricity appointment, which will focus on maintenance work at Eskom to reduce the spade of load-shading facing the country (Ramaphosa, 2023). President further explained the duties of the new minister, which emphasised the importance of smooth maintenance in power stations which will increase the company's turnover (Ramaphosa, 2023)

Once the maintenance strategy is transparent, it makes it easier for the organisation to group business and maintenance objectives (Bhebhe and Zincume, 2020).
Maintenance work's most significant advantage is regarded as a safety barrier in business operations ( Yuan, Reniers, Yang, Bai, 2023). However, with the increase in demand for product quality, firms have to adopt a maintenance strategy. To ensure plant availability, the management must re-examine the maintenance scope, including service routine, periodical inspection and quality control (T.Y. Kim, Cha, Jung, Roh, Y. Kim, Jung, Yoon, Hwang, 2020). Optimization is utilizing fewer resources to yield more products, hence in maintenance, optimization will mean cost reduction in the plant (Franklin, Snider, Tronskar, 2003). (Avish and Sanya, 2020)Agreed that the business should start focusing on equipment quality control to achieve quality products by improving equipment maintenance. Any operating organisation's objective strictly depends on plant availability and productivity to meet customer demands (Mthembu, 2015). According to (Mthembu, 2015), plan maintenance reduces plant variable and fixed costs, directly affecting business revenues. The main aim of this report is to investigate pipelines failures using radiography inspection method, in the oil and gas industry.
