Statement of the problem: A Study to assess the effectiveness of skill competency programme on cervical collar placement algorithm in cervical spine injury in terms of knowledge and practice among staff nurses in selected hospital, Meerut.

Objectives of the Study: 1. To prepare and validate skill competency program on cervical collar placement algorithm in cervical spine injury in terms of knowledge and practice among staff nurses, 2. To evaluate the effectiveness of before and after implementation of skill competency programme on improving knowledge and practice regarding cervical collar placement algorithm among staff nurse, 3. To find out the correlate between post-test level of knowledge and practice score regarding cervical collar placement algorithm among staff nurse.

Methodology: An evaluative research approach was used in the study to Skill Competency Program On Cervical Collar Placement Algorithm In Cervical Spine Injury In Terms Of Knowledge And Practice Among Staff Nurse At Selected Hospital Meerut. The research design selected for the study was descriptive design. 60 Staff nurses were selected in hospital setting by non-probability purposive sampling technique. Semi Structured knowledge questionnaire and practice checklist were used to assess the knowledge regarding clinical competency on Cervical Collar Placement Algorithm In Cervical Spine Injury Data collected and analyzed by using descriptive and inferential statistics.

Major Findings of the Study: Majority of the samples 80% staff nurses have excellent knowledge, 36% of them have very good knowledge. The majority of the staff nurses 85% of them have effective practice.

Conclusion: The study concluded that staff nurses were having Excellent knowledge and practice regarding clinical competency on cervical collar placement algorithm in cervical spine injury in terms of knowledge and practice among staff nurse at selected hospital Meerut

Cervical spine injuries often involve permanent complete or partial loss of sensory function, and many associated complications. Injuries located higher on the spine will be more severe, with high cervical spinal cord injury often being fatal. Cervical-level injury known as a C8 injury which relates to damage to the spinal cord root that exits the spinal column between vertebrae C7 and T1. The spinal cord running through the cervical region of the spine is identified by the level of the vertebra in which it’s contained. Cervical spinal cord injuries are the most severe of all spinal cord injuries and may affect one or both sides of the body. The higher up in the spine that the injury occurs, the more severe the potential outcome. Some cervical spinal cord injuries are severe enough to result in death. Injuries to C1 and C2 are very rare and most injuries to the cervical spinal column occur near the C4 / C5 levels. While no two spinal cord injuries are the same, early treatment is critical to the long-term prognosis of any injury to the cervical spinal column. STATEMENT OF THE PROBLEM:

A study to evaluate the effectiveness of skill competency program on cervical collar placement algorithm in cervical spine injury in terms of knowledge and practice among staff nurses at selected hospital at Meerut.

OBJECTIVES:

To prepare and validate skill competency program on cervical collar placement algorithm in cervical spine injury in terms of knowledge and practice among staff nurses.
To evaluate the effectiveness of before and after implementation of skill competency programme on improving knowledge and practice regarding cervical collar placement algorithm among staff nurse.
To find out the correlation between post-test level of knowledge and practice score regarding cervical collar placement algorithm among staff nurse.

RESEARCH HYPOTHESIS( at 0.05 level of significance)

H1: - There is a significant difference between post-test knowledge and practice score regarding cervical collar placement algorithm among staff nurses between experimental and control group.

H2:- There is a significant correlation between post-test knowledge and practices score regarding cervical collar placement algorithm among staff nurses.

Ethical Consideration: Ethical clearance obtained from the university ethical committee. The permission received from the concerned authority in the hospital to conduct the study. Written consent obtained from the staff nurses after explaining the purpose of the study.

Conceptual Framework: Ludwig Von theoretical framework was used in this study.

An evaluative research approach , quasi-experimental non-equivalent control group post-test design and Non – probability purposive sampling technique was used in the study. Settings - Chhatrapati Shivaji Hospital Meerut. Sample- samples are the staff nurses working in ICUs, emergency and triage at Chhatrapati Shivaji Subharti hospital Meerut. Sample Size: 60. The reliability was assessed by testing the instruments is co-relation of knowledge test was calculated by using (Kuder Richardson 20 Formula), it was found r= 0.85. It indicates tools is reliable.

The tool developed and used for data collection was a semi structured questionnaire and checklist with three sections:

Section A consists of demographic variables such as age, gender, qualification, year of experience and area of practice to know about skill competency program on cervical collar placement algorithm in cervical spine injury in term of knowledge and practice among staff nurse

Section B consists of Semi Structured Knowledge Questionnaire to assess the knowledge regarding skill competency program on cervical collar placement algorithm in cervical spine injury in term of knowledge and practice among staff nurse

Section C consists of Observation checklist to assess the skill competency program on cervical collar placement algorithm in cervical spine injury in term of knowledge and practice among staff nurse

Data Collection Procedure: The main study was conducted in ICU Chatrapathi Shivaji hospital Meerut, after obtaining permission from concerned authorities. The purpose of the study was explained to the samples and their willingness to participate was assured by getting written consent. Samples from the experimental group were selected from Chatrapathi Shivaji hospital, On the Day1,pretest was conducted by assessing the knowledge and practice on administration of drugs related to cardiovascular system and on the same day competency, the programme was given to the staff nurses working in ICU for 45 minutes by Lecturer method. On the 7^th Day, post-test was done to assess the knowledge and practice regarding administration of drugs related to cardiovascular system among staff nurse working in ICU. Samples for the Control group were selected from Chatrapathi Shivaji hospital. On Day-1 rapport maintained with the samples and the socio demographic data collected on the same day pre-test was done in Chatrapathi Shivaji Hospital. On the 7^th Day, a post-test was done to assess the knowledge and practice regarding cervical collar placement algorithm in cervical spine injury.

In experimental group 21(70%) them were in the age group of 28-32years,6(20%) of them were in the age group of 33-37 years, 3(10%) and in control group,19 (63%) of them were in the age group of 28-32years,11(36% ) were in the age group of 33-37 years. According to Gender, in experimental group 18(60%) subjects were females and 12 (40%) subjects were males and in control group 16(53%) subject were female and 14(46%) subject were males. In regard with qualification, in experimental group the majority subjects were having Diploma in nursing (G.N.M), 23(74%) subjects were having B.Sc.(N) qualification, 4(13%)subjects were Post Bsc(N)qualification 3(10%) and in control group the majority subjects were having Diploma in nursing(G.N.M),22(71%) subjects were having B.Sc.(N) qualification 4(13%) subjects were Post Bsc(N)qualification 4(13%). According to years of experiences, in experimental group the majority having 1-3 year experience, 20(66%) of them were having 4-6 year experience, 8(26%) of them were having 7-9year experience, remaining 2(6.6%) of them were having above 10years of experience while in control group the majority were having 1-3 year experience, 18(60%) of them were having 4-6 year experience 10(33%) of them were having 7-9 year experience remaining 2(6.66%) of them were having above 10 year experience. According to the area of working the data shows that in experimental group 6(20%) of them were working in Triage/Emergency, 10(33%) of them were working in RICU, 11(36%)subjects were working in MICU, and 3(10%)subjects were working in SICU and in control group 4(13%) of them were working in Triage/Emergency,8(26%) of them were working in RICU, 8(26%) subjects were working in MICU, and 10(33%) subjects were working in SICU.

The correlation between post test knowledge and practice score of nurses regarding cervical collar placement algorithm in cervical spine injury. Post test Knowledge and practice scores of nurses were analyzed by Karl Pearson correlation coefficient at 0.05 level of significance and r = 0.54 which shows that there was a moderate positive relationship between knowledge and practice in experimental group.

The experimental post test mean practice score (42.30) was significantly higher than the post test mean score (38.60) of control group with the mean difference of (3.7), the SD difference was (0.2). The obtained unpaired t test value was significantly higher i.e. 21.53 than the tabulated t-value which was at df 58 at 0.05 level of significance. To test this hypothesis, statistical test ,unpaired’ test was used. Hence research hypothesis was accepted and the null hypothesis rejected. Statistically interpreted and the skill competency program was effective.

Based on the above findings of the study following conclusion could be drawn: There was a knowledge deficit regarding skill competency program on cervical collar placement algorithm in cervical spine injury in term of knowledge and practice among staff nurse in pre test. The skill competency program on cervical collar placement algorithm in cervical spine injury in term of knowledge and practice among staff nurse was found to be effective in increasing the knowledge and practice of staff nurses.

Recommendations: This study can be replicated in large samples so that findings can be generalized for a large population. A comparative study can be done to see the difference in the effect of the assess regarding.skill competency program on cervical collar placement algorithm in cervical spine injury among staff nurse. A follow-up study can be conducted to assess the knowledge and practice of staff nurses regarding skill competency program on cervical collar placement algorithm in cervical spine injury. A study can be conducted to identify the educational need of staff nurses regarding skill competency program on cervical collar placement algorithm in cervical spine injury.
