Of 3114 included patients to study, after employing the exclusion criteria, 527 patients were excluded and 2587 patients remained for further analysis. Based on variable types (continuous, categorical), Tables 1 and 2 describe the key features of the included patients. Of the included patients, 1546(59.8%) were men and 1041(40.2%) were women. Patients' mean (SD) age was 56.8 (13) years, with a range of 18 to 93 years (P-value < 0.05). The median age of the patients was 58. 11.1% of the patients passed away in the CSICU, of which 75% had LOS \(\le\) 7 and 25% had LOS \(>\) 7 (P-value < 0.05). Aggregate mean observed ICU stay was 3.6 (5) days and the means estimated by SAPS II, SOFA, APACHE IV, and APACHE II were 4., 3.03, 5.67, and 4 days, accordingly (See Fig. 1, 2, and 3). Considerable differences have been observed for the LOS of patient with regards to age, respiratory rate, white blood cell, serum creatinine, blood sugar, gender, GCS, mortality, and mechanical ventilation. However, For the patient LOS, no significant differences in terms of other parameters were observed. More detail of clinical, hematologic and biochemical presented in the tables mentioned above and the information is categorized based on CSICU LOS days less or equal to 7 and more than 7 for further analysis.

The linear predictor formula for length of stay based on scoring systems were as follows:
With the APACHE IV and APACHE II, the R-squared was 0.005, while it was zero for the SAPS II and SOFA. F-values were 12.197, 1.263, 13.783, and 0.004 for the APACHE II, SAPS II, APACHE IV, and SOFA, in that order. Significant differences were observed for APACHE IV and APACHE II. The RMSE measure for APACHE IV (better) is lower among other models. Using AIC and BIC to compare models, shows that APACHE IV and SOFA indicates lower penalty terms hence a better model. The AUC was 0.50 (95%) for SAPS II, and SOFA, 0.54 (0.95%) for APACHE IV, and 0.75 (0.95%) for APACHE II (See Fig. 5). The ICC for the SAPS II, SOFA, APACHE IV, and APACHE II was − 0.019, -0.002, -0.042, and 0.088, respectively. The Pearson’s correlation coefficient for the SAPS II, APACHE IV, APACHE II, and SOFA were, in order, -0.37, -0.07, 0.89, and − 0.37 (See Fig. 4). The details are shown in Tables 3 and 4.

Graphical evaluation shows that in comparison with the other three models, the APACHE IV model has higher calibration over the whole range of predicted probabilities. (See Fig. 6, Table 5).
