Demographic characteristics of patients
In this study, 372 confirmed cases of leukemia identified in existing records. The median age (interquartile range (IQR)) at diagnosis was, 48 years (IQR: 24.5 – 60 years), minimum and maximum (Min-Max) age was 2-91 years and the male-to- female ratio of 210/161 or 56.6% vs 43.4%. The age-related distribution of all leukemia was 69 (18.6%) in for young patients (1–20 years), 84 (22.6%) for young adults (21 - 40 years), 122 (32.8%) for adults (41–60 years), and 160 (43%) for older adults (61 - 90 years) (Table 1). In addition, most of the patients came from the OPD, 167 (49.1%). Other patients were from the medical ward, 84 (24.7%) and the emergency ward 51 cases (15.0%).

Frequency of different types of leukemia recorded from 2010 to 2020
Table 2 presents data on the frequency of disparate types of leukemia in this cohort. Overall, 132 (35.4%) vs 240 (64.5%) had acute and chronic leukemia, respectively. The frequency of ALL, AML, undifferentiated acute leukemia, was 75 (20.1%), 41(11.0%) and 16 (4.3%), respectively. Similarly, frequency of CLL and CML was 108 (29.0%) and 132 (35.4%), respectively.

Age distribution of different types of leukemia
The overall age distribution of leukemia cases in this population of bimodal. In decreasing frequency, the estimates were in the order: 50-59 years, 63(16.9%); 40-49 years, 59 (15.8%); 60.9 years 49(13.1%); > 70 years, 49 (13.1%); 30-39 years, 44 (11.8%); 20 – 29 years, 40 (10.7); 10 – 19 years, 39 (10.5%); <10 years, 30 (8.0%). Additionally, median (IQR) age differed significantly across different subtypes of leukemia – ALL (23.0 years, IQR: 10.0 – 39.0); AML (30 years, IQR: 20 – 56 years), CLL (59.0 years, IQR: 40.75 – 66.75 years), and CML (49 years, IQR: 39.25 – 60 years), p value (Kruskal Wallis), < 0.05). No sex specific differences were observed in median (IQR) for different types of leukemia. See Figure 2 for additional information.
Hematological profiles of patients at presentation
To evaluate presentation at diagnosis, complete blood count (CBC) for selected group of patients, 102 out of 369 patients was undertaken. Due to missing data, this analysis was restricted to the following timeframe: 2016, 2017, 2019 and 2020. The median (IQR) value of the total WBC, MCV, platelet count (PLTs), and hemoglobin (Hb) at diagnosis was 50.26 x 109/L (IQR: 20 x 109 /L – 134.7 x 109 /L); 90.6 fL (86.1 – 97.8 fL); 97.0 x109/L (27.5 x 109 - 270.25 x 109) and 10.0 g/dL (7.8 - 11.0 g/dL), respectively. Overall, 62% of the patients were anaemic at diagnosis (Hb < 10g/dL), while 51.6% were thrombocytopenic (platelet count < 100 x 109/L). As seen in Fig 3, significant differences (p-value < 0.05) in median (IQR) values were observed in disparate subtypes of leukemia. On analysis of symptoms and signs, it was observed that fever was the commonest symptom, followed by weakness, bleeding and bone pain. The incidence of hepatomegaly and splenomegaly was common mainly in CML and CLL.

The cumulative CIR, ASIR and 10 year - EAPC for the population under study were 9.22 per 100 000 persons, 30.1 per 100 000 persons and – 1 (-6.1 – 6.7), p-value 0.9. Disaggregation of CIR, ASIR and AAPC data by sex was undertaken. In this analysis, estimates indicated a CIR of 10.46 per 100 000 persons in males and 8.92 per 100 000 persons in females. Associated ASIR were 18.3 per 100 000 (2010 – 2020 AAPC 95 CI: - 2.1 (-5.7 – 10.6), p value 0.56) in males vs 12.06 per 100 000 persons (2010 – 2021 EAPC 95% CI: - 3.2 (-10 – 4.1), p-value 0.035) in females.
Crude incidence rate and age-standardized incidence rates for subtypes of leukaemia diagnosed in Eritrea: 2010-2020
The CIR, ASIR and EAPC over the study period (2010 – 2021) were as follows: 2.01 per 100 000 and 3.87 per 100 000, EAPC, 21.9%(95 CI: 3.1-44.1%), p value = 0.025 for ALL; 0.94 per 100 000 and 2.38 per 100 000, EAPC, 14.9%, p value = 0.57, for AML; 2.94 per 100 000 and 15.37 per 100 0000, EAPC, -2.1%, p value = 0.65, for CLL; 3.61 per 100 000 and 24.03 per 100 000, EAPC, -0.2%, p value = 0.95 for CML; and 9.68 per 100 000 and 30.5 per 100 000.
