Ethics approval was granted by the French regulatory board (Comité de Protection des Personnes Ile de France XI, CPP number 12018) on 14/05/2012 and authorised by the Agence Nationale de Sécurité du Médicament et des produits de santé on 05/04/2012 (Supplementary appendix). The trial was conducted in accordance with the Helsinki Declaration (32) and was prospectively registered in the ClinicalTrials.gov registry (NCT01791868). Written informed patient consent was obtained by the investigator of the participating centre (supplementary appendix).
