Appendix 1 Interview Guide
Rationale: The purpose of the semi-structured interviews is to understand, from the patients’ perspective, the 1) feasibility, 2) acceptability, and 3) impact of the CHC Community Health Worker Program piloted at the Meriden clinical site. Interviews with patients currently enrolled in the CHW pilot program (N=31) will be conducted at the conclusion of their engagement in the 6-month intervention. The research team will then triangulate data from the semi-structured interview described here with data collected at baseline from the “Meet the Patient” assessments, to draw conclusions about the CHW pilot program. To our knowledge, this is the first evaluation of a U.S.-based CHW program grounded in the experiences of patients.
